In this case, we use:# 'は' ('wa') in ---> わたしは（ｎａｍｅ）です。
# 'こちら, さん and です' in ---> こちらは（ｎａｍｅ）さんです。
When given an example:
A is B = A は B です.
A's B = A の B.
Is A, B? = A は B ですか. (Adding 'か' at the back of sentence means a question.)(Thus '?' is removed)

The image you scan and post here very blur cant see the char well anyway to improve it?
From wat i know, the picture can be enlarged by clicking on it.
